      1  Do you accept the responsibility for your success and/or failure on a test rather than blaming results on other factors (like your  prof, your job, your family, your friends, your schedule)?
      2. Do you accept responsibility not only for your actions but for the consequences of your actions?
      3.  Do you take it  upon yourself to make sure you are on time for each class?
      4.   Do you demonstrate maturity, courtesy & professionalism by emailing your prof when you are absent. In this email do you make arrangements with an instructor to get your homework in on time if you are  going to be absent or to take a test early  if you will be absent during the test?
      5.Classroom behavior: Do you take it upon yourself to ask questions in class if you do not understand? Do you listen respectfully to others' questions. Do you avoid whispering to your neighbor in class.  Is your posture respectful with your head upright & not on your desk? 
      6.  Are you responsible in all your conversations (not regretting what you say at a later time)?
      7. Are you responsible in your time management skills? (planning of studying time, leisure, family time, errands...)
     
8.  Do you say NO to a friend, with a smile, when you are just too busy?
      9.  Are you responsible in your  health habits (nutrition, sleep,exercise vitamins...)
      10.  Do you use the word "choose" rather than have to?
 (Example, I choose to take math 101; I choose  to make up that test; I chose to study hard...)

Score:  Give yourself 10 points for each yes; 5 for each sometimes; & 0  for each no.  
A score of 90% and up indicates you're responsible

I didn't do  well on the test.  My brother had ESPN on too loudly. 

Journal Entries:
1.  Record your thoughts on the following:  In which ways am I most responsible?  In which ways am I least responsible? What am I doing that doesn't work? What specific steps (did I and )will I take this week  to show that I am responsible?

2.  Also,  do you choose to attend class ,do your assignments, & be in college, or do you feel you have to?

3.  Provide 5 different endings to the following sentence stem:

       When I choose to act responsibly, I..........................................

4.  Draw a vertical line in the middle of a sheet of paper. 
On the left side record behaviors that show lack of responsibility & appreciation in the classroom.
On the right side, describe behaviors that typify a mature, responsible & appreciative student.

 

5.  Write a commitment to yourself.
